This
Department provides comprehensive social services to the residents of
New Brunswick in areas including health, recreation, and vital statistics.
The Department is made up of seven divisions:
Health

In conjunction with the Middlesex County Health Department,
The Division of Health provides environmental health investigations and
inspections of all licensed food vendors in accordance with Chapter 12
of the New Jersey Sanitary Code.
The
Division also coordinates with the Middlesex County Department of Health
and the New Brunswick Schools to provide immunizations, health screening
sessions and health education programs throughout New Brunswick.

Recreation 732.745.5125
The Division of Recreation administers various programs and services geared
toward residents of and visitors to the City of New Brunswick. Programs
include adult and youth athletic leagues in soccer, softball, baseball,
and basketball, seasonal events including the Raritan River Festival and
Summer Concert Series, and organized field trips.
In
cooperation with the Division of Parks, The Division of Recreation schedules
and monitors the use of all parks throughout the City of New Brunswick.
The
Division of Recreation also manages the HUB Teen Center located at 411
Joyce Kilmer Avenue. The Center provides New Brunswick residents ages 13-19
with a place to join together outside of the traditional school hours.
Homework assistance, computer labs and training, professional development
skills workshops, and recreational programming are just a few of the amenities
offered to members of the Teen Center.

The
New Brunswick Senior Citizen Resource Center (NBSCRC) is a multi-purpose
facility for independent seniors and is designed to provide a safe,
supportive and stimulating environment. All New Brunswick residents
sixty years of age and older are encouraged to become members. Operation
hours are Monday through Friday from 8:30 am to 4:30 pm. Serving a
diverse population, the NBSCRC offers a variety of cultural, social,
educational, recreational and health related programs designed to
enhance the quality of life of New Brunswick's senior community. The
NBSCRC also provides social service information/assistance, a breakfast
and lunch program and free transportation. There is no fee for
membership.

Responsible for registering all births, civil unions, domestic
partnerships, marriages and deaths in the City of New Brunswick.
The following information is pertains to Vital Statistics:

Certified copies of births, civil unions, domestic partnerships,
marriages, and deaths
• Applications for civil unions and marriages
• Filing of domestic partnerships
• Modifications of all vital records
There are three ways to obtain certified copies:
•
The fee for all certified copies is $20.00 each.
• In person, our address is 78 Bayard St (Bayard St. is off of
George St.; City Hall is on the left hand side after the Commerce
Bank. Parking is behind the building. There is a guard at the
security booth, tell him you are going to the Health Dept and he
will direct you to our office). Acceptable methods of payment are
cash, money order, certified check, or debit cards with an
additional fee of $2.50.
• By mail: The mailing address is: New Brunswick Health Dept., P.O.
Box 269, New Brunswick, NJ 08903-0269. Acceptable methods of payment
are money order or certified checks made out to the City of New
Brunswick. Credit cards w/ expiration date are acceptable with an
additional cost of $6.00 charged through your credit card. Photo
copies of ID must be submitted along with request.

Eligible individuals who can obtain certified copies of vital records.
The individual of legal age, the parents of, a brother or sister of
legal age (with proof of their relationship i.e. certified copy of birth
certificate showing parents names), the spouse/partner (with proof of
civil union, domestic partnership or marriage certificate), the child of
legal age (with proof of their relationship i.e. certified copy of birth
certificate showing parents names), the grandchild of legal age (with
proof of their relationship i.e. certified copies of birth certificate
showing parents and grandparents names), the legal
guardian/representative with proper court documents within a 2 year
period).
Required Identification.
•
A valid photo drivers license showing your CURRENT address and
signature;
• If a valid photo drivers license is not available, two of the
following are necessary:
• A non-photo drivers license
• County ID
• Passport
• Vehicle registration and/or insurance card
• Copy of utility bill, bank statement
• Proof of signature is required

beginning of the Department of Social Services web page.
When filling out request form, the information provided must be exact
and complete as it appears on the original record. Please provide a
phone number where you can be reached should a problem arise.
If you wish to apply for a civil union/marriage license the following is
needed:
To apply in person:
•
Application can be made where either party resides with proof
• Both parties may be present; if applying separately, the same
witness must be present
• 18 years of age
• Provide social security numbers (which will be kept confidential)
• Witness 18 years of age or older
• Fee of $28.00
Suggested additional items :
•
Birth certificate, drivers license and/or passport
• If you were in a former union, and it was legally dissolved, final
decrees
• If by death, certified copy of death certificate
You may also complete the applications on the State of New Jersey’s
website at
www.state.nj.us/health. You must follow the instructions on this
site. There is a three day waiting period before the license can be
issued. If a time waiver is necessary, it must be granted by a Superior
Court Judge.
To complete a Domestic Partnership:
•
Each partner must sign in front of a notary an Affidavit of Domestic
Partnership (Affidavits are available from any local registrars
office)
• Partners must be 62 years of age
• Share a common residence or member of NJ State Retirement System
• Jointly responsible for each others common welfare
• Not in any other commitment recognized by NJ Law
• Notarized Affidavit must be taken to any local registrars office
to be filed and purchase a certificate of Domestic Partnership
• Fee of $28.00

The City of New Brunswick provides transportation for all eligible
residents to doctor's appointments and social service agencies within
the Greater New Brunswick Area.

Youth Services System
New Brunswick's Youth Services System provides comprehensive
after school programming for New Brunswick residents ages 6-15. This program
operates at seven school within New Brunswick providing the opportunity
for children from all parts of New Brunswick to participate at a site
close to their home.
Activities
provided in through the Youth Services System include homework assistance,
peer counseling, leadership training, athletic programming, and workshops
in music, drama, and photography.
Parents
and other family members are encouraged to take part in much of the programming.
The Youth Services System strives to bring together business leaders,
social service agencies, community groups, and educators in an effort
to offer programming which will be most beneficial to participants.
The
Youth Services System has also formed partnerships with several departments
at Rutgers University to provide educational, training, and internship
opportunities for Rutgers students as well as leadership, staffing, and
financing for YSS participants.

Animal

The New Brunswick Division of Animal Control works in a coordinated
effort to protect both residents and pets and stray animals. The Division
responds to calls in both Highland Park and New Brunswick, rents traps
to New Brunswick residents to remove unwanted animals from private property
in a harmless way, and provides basic veterinary services to animals without
owners.
In
accordance with Municipal Codes 6.04 and 6.08 the Division of Animal
Control licenses all pets and exotic animals kept in the City of New Brunswick.
To apply for a pet license fill out the appropriate form and return it
with payment, no personal checks to the Department of Health office at:

The
Division also offers free educational information to residents on issues
related to pet care and handling as well as rabies and neuter/spay programs.
